Products, Inc. believes in and respects your privacy and the sanctity
of information that may be provided. Further, we believe the use
of information should be outlined for you as we have done so below.
Please read the following policy to understand how your personal
information will be treated as you make full use of our many offerings.
This policy may change from time to time, and you will be notified
upon logging in of any changes – otherwise your acceptance
of these terms will apply to your future participation in Premier
Health Products services.
The information we collect is typically user-generated video
content, however may also include text entries and ratings of
concepts, and is used in aggregate form to help us understand
marketplace needs as well as to create and deliver better products
and services. Premier Health Products does not sell or rent any
personally identifiable information.
Premier Health Products also utilizes participant video submissions
as customer testimonials on websites as well as providing that
content to those companies who market and distribute our products.
The effective date for this Privacy Policy is November 5th, 2008.
What personally identifiable information does Premier
Health Products collect from me?
In the course of recording or uploading video, you may have the
opportunity to voluntarily submit personally identifiable information
(e.g., providing profile information or providing an e-mail address).
T. Harmon, Inc., maintains this information in a database on behalf
of Premier Health Products, and may use this information to invite
you to participate in future projects, or communicate with you
regarding projects in process in which you are participating,
subject to agreement with the authoring and the respondent-providing
client. Your decision to participate in future projects is completely
at your discretion. That is, you have no obligation to complete
this or any future project.
If you contact Premier Health Products we may keep a record of
that correspondence. Wherever Premier Health Products collects
personal information we make an effort to include a link to this
Privacy Policy on that page. By participating in a project or
in visiting the Premier Health Products Web site, you understand
and consent to this policy. Who is collecting information and
for what purpose?
Premier Health Products collects information which assists in
developing new products, services, and other research. When you
are on a Premier Health Products site and are asked for personal
information, you are sharing that information with Premier Health
Products and T. Harmon Inc alone, unless it is specifically stated
otherwise. If data is being collected and/or maintained by any
company other than Premier Health Products and T. Harmon, Inc.
you will be notified prior to the time of the data collection
or transfer. If you do not want your data to be shared, you can
choose not to allow the transfer by not using that particular
service.
Any Sensitive Information (i.e. personal information specifying
medical or health conditions, or information specifying the sex
life of the individual) will be treated with additional care.
If Premier Health Products or T. Harmon, Inc. intends to disclose
such information to a third party or use it for a different purpose
than originally collected, then you will be expressly asked permission
prior to any such disclosure. What security precautions are in
place to protect the loss, misuse, or alteration of my information?
Also remember to sign out/log off of your Premier Health Products
account and close your browser window when you have finished your
work. This is to ensure that others cannot access your personal
information and correspondence if you share a computer with someone
else or are using a computer in a public place like a library
or Internet cafe.
Whenever Premier Health Products handles personal information
as described above, regardless of where this occurs, Premier Health
Products takes steps to ensure that your information is treated

Notice to Children and Young People
If you are under the age of 13, you may not use any services
of our site, or participate in any video diary projects.
Incentives
Periodically we request information, typically e-mail address
or your mailing address if needed to mail a web cam or participation
incentive to you specific to a particular project. Participation
is completely voluntary and users may choose whether or not to
disclose this information. Users’ personally identifiable
information is not shared with third parties unless we give users
notice and choice. Although we may use an intermediary to fulfill
incentives, they may not employ users’ personally identifiable
information for any secondary purpose.
Use of Cookies
As part of offering and providing customizable and personalized
services, Premier Health Products may use cookies to store and
sometimes track information about you. A cookie is a small amount
of data that is sent to your browser from a web server and stored
on your computer’s hard drive. Some areas on Premier Health
Products where you are prompted to log-in or that are customizable

provider (ISP), referring/exit pages, platform type, date/time
stamp, and number of clicks in order to administer the site, track
aggregate movement, analyze trends, and gather broad demographic
information for aggregate use. This information is not linked
to personally identifiable information.
Security
We take every precaution to protect our users’ information.
When our users enter sensitive information (such as credit card
information), that information is protected using SSL encryption.
While on a secure page (such as our Order Form) the lock icon
on the bottom of most Web browsers such as and Microsoft Internet
Explorer or Firefox becomes locked. Users who have any questions
about the security at our Web site are encouraged to contact us
as indicated below.
Confidentiality
We are sensitive to the privacy and security of your personal
information. However, we cannot guarantee absolute security as
such information may be transmitted using the Internet, other

- If a user wishes to subscribe to our newsletter, we request

request an e-mail address. Out of respect for our users'’
privacy, we also enable users to opt-out of future project invitations
by following the Opting-Out practices outlined below.

- On rare occasions it may be necessary to issue a service related
announcement (for instance, if our service is undergoing maintenance).
These communications are not promotional in nature and users
may not opt-out of these communications (although users can
deactivate their account).
- Customer Service Communications
- At a user’s request, we may communicate via e-mail,
chat, or telephone to answer service-related questions. E-mails
and/or chats transcripts may be maintained by Premier Health
Products to provide assistance in resolving any inquiries.
Opt-Out Policy
Users who no longer wish to receive our newsletter, promotional
communications, or project invitations may opt-out by replying
to any received e-mail with “remove” or “unsubscribe”
in the subject line or by contacting us as indicated below.
Maintaining, Updating, Deleting, or Deactivating Personal
Information
If a user’s personally identifiable information changes
(such as zip code, phone, e-mail or postal address), or if a user
no longer desires our service, we provide a way to correct, update
or delete/deactivate users' personally identifiable information.
This can be done by e-mailing Premier Health Products Customer

below. Premier Health Products will respond to any access request
provided within 30 days of submitting the request

that we, Premier Health Products, are not responsible for the
